Why alcohol affects women differently to men

Pour a man and a woman of the same weight the same three drinks and they don't end up in the same place. The short answer

After the same amount of alcohol, a woman's blood alcohol level tends to climb higher than a man's — even accounting for body size. Two things drive it:

Reason one: less body water

Alcohol is water-soluble. Once it's absorbed, it distributes itself through your total body water — the more water there is, the more diluted it becomes. On average, women's bodies are a lower percentage water than men's of the same weight, and a higher percentage fat, which holds very little alcohol.

Why "I can keep up" is a trap. Matching someone else drink for drink assumes the alcohol does the same thing in both bodies. It doesn't. A woman keeping pace with a larger male friend can be a full standard drink's worth of BAC ahead by the end of the night without touching a single extra glass.

Hormones and other factors

Body water and enzymes are the big two, but they aren't the whole picture. Hormone levels shift across the menstrual cycle, and some people notice alcohol hits harder at certain points — though the effect is individual and hard to generalise. Where you are in your cycle, whether you've eaten, how quickly you're drinking and your overall body composition all move the number. The honest takeaway is that averages describe populations, not you: your own experience over time is the better guide.

The longer-term health picture

The differences aren't only about a single night. Because women reach higher blood alcohol levels and their tissues are exposed to more alcohol per drink, research links alcohol-related harms — including to the liver, heart and certain cancers — to lower levels of drinking and shorter timeframes in women than in men. It's one of the reasons "the less you drink, the lower your risk" applies to everyone, and lands a little sooner for women.

But aren't the guidelines the same?

They are — and that surprises people. The National Health and Medical Research Council (NHMRC) advises healthy men and women alike to have no more than 10 standard drinks a week and no more than 4 on any one day. The single set of numbers reflects long-term disease and injury risk across the population, not how quickly an individual feels drunk on the night.

So the guideline being identical doesn't mean the experience is. A shared "4 in a day" limit still leaves a woman at a higher blood alcohol level than a man who drank the same four. The Alcohol and Drug Foundation stresses these are guidelines for reducing risk, not targets — and the less you drink, the lower your risk over time. Separately, the guidelines are clear that women who are pregnant or breastfeeding should not drink at all.

Don't rely on "the same as him" to drive. Standard-drink counts and BAC estimates are exactly that — estimates — and your body clears only about one standard drink an hour regardless of sex.
